* Please note that applicants who buy an EC unit from the developer, will be considered as having taken one housing subsidy, regardless of whether thay are eligible for or whether they would take the CPF Housing Grant. They would have to pay a resale levy, when they apply to buy a second subsidised flat in the future, depending on the prevailing policy then.

I/We confirm that prior to *my/our booking of the EC unit, the applicants/*co-applicants/*occupiers listed in *my/our EC applications,
i. bought, taken over ownership (by way of gift or otherwise), acquired an interest in, sold or transferred es (by way of gift or otherwise) a flat bought directly from HDB, resale flat bought under the CPF Housing Grant scheme, DBSS flat or an EC unit from a developer. ii. taken other forms of housing subsidy (such as benefits under the Selective En bloc Redevelopment es Scheme (SERS), privatisation of HUDC estate etc.). iii. unlocked part of the housing equity under the Lease Buyback Scheme (LBS) or transferred a flat under the LBS. iv. surrendered a Studio Apartment or a flat previously bought from HDB, an HDB Resale flat bought under the CPF Housing Grant scheme or a DBSS flat bought from a developer. Please delete whichever is not applicable as indicated by an * (asterisk) below. *I/We declare that :- 1. All the particulars given in this Application Form are true and correct, and *I/we have not suppressed any information required in the Form. 2. Neither *I/We, nor any person listed in this application have any outstanding debt ** due to the Housing & Development Board (hereinafter called the "HDB"). **If the applicant is an existing owner of an HDB / DBSS flat, "debt" herein does not include the existing mortgage loan granted on the HDB / DBSS flat. 3. *I/We *am/are not an undischarged bankrupt. 4. *I/We have read and understood and agree to abide by the Terms And Conditions Relating to the Application For The Purchase of A Housing Accomodation Under The Executive Condominium Scheme. *I/We fully understand and accept that the Minister for National Development and/or the Developer shall have the right to unilaterally introduce new Terms And Conditions Relating to the Application For The Purchase of A Housing Accomodation Under The Executive Condominium Scheme and/or vary the terms and conditions thereof at any time, even if such new or varied terms may adversely affect *my/our eligibility to apply for an EC unit. 5. *I/We will notify the Developer immediately in writing of any changes in the information supplied in this Form and any changes affecting *my/our eligibility for the purchase of an EC unit. Prior to *my/our taking possession of the EC unit, *I/we will also complete and return to the Developer the Further Particulars Form declaring if there is any change affecting *my/our eligibility. *I/We understand that in the event of any change, HDB has the right to reassess and reconfirm *my/our continued eligibility before *I/we may take possession of the EC unit. *I/we must continue to maintain this eligibility throughout our occupation of the EC unit. 6. *I/We understand and accept that :- a) The Developer has the right to reject/cancel *my/our application without assigning any reason whatsoever if :- I) II) *I/We *am/are deemed to be /have become ineligible to purchase an EC unit; or *I/We do not pay up the 5% booking fee at the time of booking of an EC unit; b) If any information given in this Application Form is incorrect or if *I/We have suppressed any information required in the Form, *my/our application shall be cancelled. The Developer or the appropriate authorities, including Minister for National Development and/or HDB, will be entitled to take the relevant action under the Option to Purchase/Sale & Purchase Agreement and/or Executive Condominium Housing Scheme Act. *My/Our application and statements and representations therein made to the Developer would be forwarded, and be deemed to be made by *me/us, to the Minister for National Development and/or HDB who shall be entitled to take such action as it may be necessary including but not limited to enforcement action (including compulsory acquisition) based on any misleading or false statement or misrepresentation. c) If * I/We have made any false statements in the application, HDB may debar me/us from buying a flat* or taking over ownership of a flat* by way of transfer, being included as an essential occupier of a flat* or taking up an HDB housing loan for a period of 5 years from the date of cancellation of the application or the date of approval to compulsorily acquire the EC unit. Any existing application registered in the HDB's rental waiting list will also be cancelled. *includes a unit to be bought under the Design, Build and Sell Scheme or the Executive Condominium Housing Scheme. d) The Developer has the right to conduct such further checks to verify and confirm *my/our eligibility and the eligibility of the persons listed in the application, including but not limited to requiring *me/us to submit such documents as and when they require. 7. *I/We hereby agree and confirm that should *my/our application be successful, the Terms And Conditions Relating to the Application For The Purchase of A Housing Accomodation Under The Executive Condominium Scheme shall form part of the terms and conditions of the developer's approval for the purchase of an EC unit. 8. *I/We undertake to abide by the Minister for National Development's and/or its appointed agents' and/or the Developer's policies, terms and conditions relating to the Executive Condominium Scheme laid down from time to time. 9. *I/We hereby unequivocally and irrevocably authorise the Developer to furnish any information whatsoever regarding *my/our application to the Minister for National Development and/or its appointed agents for the purpose relating to or in connection with this application. 10. *I/We understand and agree that Developer and the relevant authorities including HDB shall have the right to rely on its/their own records and/or the records of any other relevant authorities to obtain or verify any information relating to or in connection with this application. In this regard, Developer and the relevant authorities including HDB also reserve the right to request any information from all or any of the applicants without informing the other applicants. 11. If I*/We *am/are the existing lessee(s) of a HDB flat or a DBSS flat, *I/We hereby undertake to sell/transfer/relinquish *my/our existing flat within six months from *the date of issuance of Temporary Occupation Permit in respect of the Executive Condominium Unit where the Temporary Occupation Permit has not been issued or the date of the Developer's Notice of Vacant Possession of the Executive Condominium Unit where the Temporary Occupation Permit has been issued or such other period as may be stipulated by the HDB in writing.
